Homes4Hope offers ASAM 3.1 Low-Intensity Residential Treatment, which is a level of care defined by the American Society of Addiction Medicine (ASAM) for adults in Salisbury, MD. This program is designed for individuals who are transitioning from higher levels of residential care or those who are unable to stop using drugs or alcohol in lower levels of care because of unstable and unsafe social environments.
Our licensed residential facilities offer round-the-clock support and comprehensive treatment. Clients reside on-site and engage in psychiatric care, counseling, group meetings, life skills classes, recreational activities, and more. Following discharge, we assist with continuing care planning and coordinate outpatient services. Residential treatment enables individuals to concentrate fully on their recovery journey.
An Overview of Our ASAM 3.1 Program
Our ASAM 3.1 programs offer an average stay of 6 to 12 months, which is tailored to your treatment plan and progress. During a typical week, you can expect to receive 5-10 hours of clinical care in individual or group settings, along with individual and group sessions with peer specialists. We also provide transportation to outpatient mental health programs, as well as access to other supportive partner programs. In their free time, clients have the opportunity to attend support groups, volunteer, work (when appropriate), and schedule time with family.
Treatment Philosophy
Our ASAM 3.1 program is staffed with clinicians who specialize in treating substance use disorders, as well as peer specialists who model recovery and provide support for clients in early recovery. Additionally, we have behavioral technicians who oversee the housing component and daily routines of all our clients.
Our clients also have access to mental health therapists and psychiatric nurse practitioners at our outpatient offices, who offer psychiatric medication management and medications for addiction treatment.
Community involvement in recovery is vital for sustaining recovery. Therefore, our clients are expected to attend support groups outside the house and participate in other recovery-oriented social activities.
Eligibility Criteria
In order to be admitted into our ASAM 3.1 program, individuals must undergo an assessment to determine if they meet specific criteria for this level of care. Generally, those at this level of care do not require withdrawal management, have few or well-controlled medical problems, are not experiencing any acute psychiatric conditions, and are living in unsafe environments that are not supportive of recovery.
It is essential for the individual to recognize the need for this level of care at their current stage of recovery. Acknowledging the importance of actively participating in the therapeutic environment at their living space is crucial.

Our spacious homes have bedrooms that can comfortably accommodate 1-2 other clients, promoting a sense of accountability, structure, and support crucial for early recovery. Each residence is equipped with communal living spaces, including a kitchen, dining room, and living room, as well as multiple restrooms.
These shared areas are designed to create a cozy, homelike atmosphere, allowing clients to not only receive clinical care but also develop important life skills. Activities such as cooking, cleaning, and maintaining a home are integral to our program, helping clients rebuild their independence and confidence in a supportive environment.

About Individualized Treatment Plans
Our services are designed to meet the needs of both individual clients and groups. We provide personalized attention through small caseloads, ensuring each patient receives a customized treatment plan. This plan is carefully developed following a detailed assessment.
The assessment covers various aspects of the patient’s life, including physical health, emotional well-being, and mental health. It also involves a thorough review of their medical and substance abuse history, family background, and other relevant personal factors. This comprehensive approach allows us to address the unique recovery needs of our clients, facilitating a more effective and holistic healing process.
Integration of Other Medical Services
We have partnerships with other community providers so we can meet the needs of clients at varying stages of treatment. We assist our clients with arranging necessary appointments and procedures, including laboratory and toxicology tests, as appropriate to the severity of the patient’s condition.
Level 3.1 clients may require services such as dental care, laboratory testing, or toxicology, which can be accessed through affiliation or arrangement with qualified providers in the community. The program also has the ability to arrange for psychiatric medication management or medication assisted treatment.
Research indicates that appropriate use of pharmacotherapy can benefit clients and improve treatment outcomes at Level 3.1. These services can be assured through relationships or affiliations with community providers, with facilitated access via transport assistance.
Transition and Aftercare Planning
In our program, every client works with their counselor and peers to achieve their individual goals. We understand that each person’s journey is unique, which is why we offer a transition and aftercare plan specifically designed to meet each client’s needs.
This ensures a personalized approach to recovery and support. To monitor progress and adjust plans as necessary, assessments are regularly scheduled through our outpatient office. Program Costs and Insurance Plans
Our contract with Maryland Medicaid for ASAM 3.1 programs ensures that the entire cost of treatment and residential care is covered, offering substantial financial relief to our clients. This coverage includes all necessary treatments and accommodations.
While basic nutritional needs are provided for, clients must purchase any extra food items beyond the standard provisions. Additionally, personal items or any extras for comfort or convenience during their stay are the patient’s responsibility.
